This form makes it easy to order a replacement ID card for a student or staff member who has either lost, damaged or received an ID card with the wrong information.
If the information is incorrect just fill in the appropriate box (i.e. last name) with the correction information and the form will automatically update our production software. Please remember, the new information will appear exactly as you typed it…be sure to use capital letters where necessary.
